I just created a little fake data set and it worked fine for me:

. gen Include=0

. replace Include=1 if Temp==42.5 | Dose_Group<3 (3 real changes made)

What version of Stata are you using? Is it totally up to date? Perhaps parentheses would help -

replace Include=1 if (Temp==42.5 | Dose_Group<3)
